The Diablo 4 Holy Chalice bug is a cursed thing to come across. If you’re working on any of Sister Octavia’s quests out of Kyovashad, you may have come across it. What is the Diablo 4 Holy Chalice Bug? — Explained

The Diablo 4 Holy Chalice bug has two versions.

  • The Holy Chalice used by Sister Octavia is stuck in your inventory, even when it shouldn’t be there.
  • The Holy Chalice shows up at the gates of Kyovashad, and picking it up does nothing once you leave the game. Once you load back in, the chalice is back in front of the Kyovashad gates.

The Holy Chalice bug seems to affect other quest items as well, causing them to spawn at the gates if you picked up a chalice someone else dropped there. From what I’ve seen, this bug triggers when completing the questline in different tiers, as well. If you complete Malady of the Soul at World Tier 1, you’ll need to complete Depths of Despair and Faith in Blood at World Tier 1, not anything higher.

The best way to avoid the Diablo 4 Holy Chalice bug is to avoid changing World Tiers and complete all of the quests from Sister Octavia in the same World Tier. The quests are:

  • Malady of the Soul
  • Depths of Despair
  • Faith in Blood

How to Fix the Holy Chalice Bug in Diablo 4

If you have changed World Tiers and/or are running into the issue, there’s really only one way to potentially fix the Diablo 4 Holy Chalice bug.

  • Portal to the Kyovashad waypoint.
  • Speak to the NPC quest giver near the portal.
  • Complete the quest.

If this doesn’t work for you, you’ll need to wait until Blizzard patches the game with a fix for the Holy Chalice bug. You can ignore the chalice at the gates and hopefully have enough room in your inventory to have it not be as noticeable. The best course of action is to complete all three quests one after the other on the same World Tier as quickly as possible.
