Diablo 4: How to Get Distilled Fear

Distilled Fear is one of the hardest new items to obtain in Diablo 4 Season of Blood.

As part of the Season of Blood content in Diablo 4, you’re able to acquire the new item Distilled Fear, which is a necessary component for summoning Beat in Ice. In this guide we’ll go over how to get Distilled Fear in D4.

How to Get Distilled Fear in Diablo 4

To obtain Distilled Fear, you need to complete Tier 30 or higher Nightmare Dungeons on World Tier 4. Not only do you need to have Tier 30+ sigils drop or get the required items to craft them, you then need to complete the dungeons multiple times. Distilled Fear isn’t a guaranteed drop and I’ve found that it can’t simply find in a chest in a set location like Living Steel.

To gather all the materials needed, you need nine vials of Distilled Fear. That means you need to complete at least nine Nightmare Dungeons over Tier 30. For those of us with limited time, like myself, selecting the shorter dungeons can make this grindy process at least a little more bearable. Short dungeons include:

  • Guulrahn Canals
  • Aldurwood
  • Blind Burrows
  • Maulwood
  • Witchwater

Guulrahn Canals is the shortest and easiest to complete dungeon, making it ideal for farming Distilled Fear.

Distilled Fear Use

Distilled Fear is needed if you want to battle the new endgame boss, Beast in Ice. Unlike the other endgame bosses introduced in Season 2: Season of Blood, you need to do some crafting before you can face this icy enemy. You need a total of nine Distilled Fear, which is then turned into a Glacial Fissure Nightmare Sigil at an Occultist. From there you can use the sigil and head to the dungeon.
