One of the most important collectibles in SpongeBob SquarePants: The Cosmic Shake comes in extremely limited quantities. But that’s because the Golden Underwear are by far the most useful collectible in this throwback-style 3D platformer.
In SpongeBob SquarePants: The Cosmic Shake, your health is measured in underpants. You start with four by default, but finding each pair of golden underwear will earn you one more underpants (hit point) to your overall health bar. There are only two golden underpants to collect in the entire game. Here is where to find each of them.
Where to Find the Golden Underpants in Karate Downtown Bikini Bottom

You can actually pick both of these collectibles up on your first time through each of these worlds. This first one is in Karate Downtown Bikini Bottom.
At the start of the Paparazzi Street segment, go right through the hole in the fence. Jump on the dumpster here to get up on top of the brick wall and walk to the pillar platform.
From here you will see an inflatable platform you will need to use your bubble wand on. Once inflated, cross over to the small circular platform on the other side and on to the nearest rooftop via another platform.
From here you can see the underpants you seek. The golden underpants are on the platform across the way, which you can now get to by gliding and then using the karate kick balloon that is right near the health upgrade.
Where to Find the Golden Underpants in Medieval Sulfur Fields

The second health upgrade is found in the Medieval Sulfur Fields in the Castle Courtyard. In this area, you will see the underpants behind an unbreakable wall of blocks. So how do you get in?
For starters, you’re going to want to climb up the ramparts. Continue up the seesaw platform, and onto the the section with spike traps. Now look down on the outer side of the rampart wall and you will see floating tiki boxes that lead down to a ledge where an entryway is blocked by boxes.
Jump over to the platform moving in and out of the castle spire, and then hop down to the tiki platforms. Once you’ve landed on the ledge, attack the boxes and you will be in the blacksmith shop. Here, you can pick up the golden underpants and collect your final health upgrade.
